For professionals

Make a referral

Social workers, EDT, schools and youth offending teams: tell us what you need, and a member of our team will call you back within 24 hours to talk it through.

How it works

  1. You send an enquiry

Your details and the service you are looking for. No details about the young person.

  2. We call you back

We aim to contact you within 24 hours, then take the young person's details securely.

  3. Support starts

We match the young person with a keyworker, agree a plan with you and the family, and report back as we go.

Referral enquiry

Please do not include the young person's name, date of birth, address, school or case details. This form is only about you and the support you are looking for. We will call you back and take the young person's details through your council's secure channels.

For example: social worker, EDT practitioner, SENCO, YOT officer

Frequently asked questions

Who can make a referral?

Professionals working with a young person or family: social workers and children's social care teams, the Emergency Duty Team, schools, youth offending teams and other services. We work with Hackney, Greenwich and Lewisham councils.

Why can't I include the young person's details?

A website form is not the right place for sensitive information about a child. We keep this first step to your contact details and the service you need, then take the young person's details by phone or through your council's secure channels.

I am a parent or carer. Can I refer my child?

Our support is arranged through children's social care. Please ask your social worker to contact us. You are welcome to ask us a question first.

Can I get help tonight?

Our out-of-hours crisis support is for families registered through the councils we work with. Council staff can find the details in the council portal. If anyone is in immediate danger, call 999.
